Accommodation Request Process

Student Steps 

  • Complete this form and Submit. 
  • Submit documentation supporting the accommodation request. 
  • You can attach your documentation below, submit your documentation to the CDS Office (Building S, room S114) in person, or you can fax it to 708-974-8330.
  • Schedule an intake meeting with a CDS staff member. 
  • To schedule a meeting you can call 708-974-5711, stop by the office (S114) or email us at disability.services@morainevalley.edu

CDS Steps 

  • Following the intake meeting, CDS staff will review the information you have shared and determine the accommodations that can be approved. 
  • CDS will notify the student about the decision and prepare accommodation letters for the student to present to their instructors. 

Accommodation Letter Updates or Revisions: 

Students can request updated or revised accommodation letters at any time by completing an Accommodation Update in Accommodate and scheduling a meeting with a CDS staff member. Depending on the request, CDS may request that the student provide additional supporting documentation. CDS will review the request and notify the student about the decision. 

Confidentiality: 

CDS is committed to protecting the privacy of students and will not release information from a student’s educational record without the student’s written consent, except in the limited circumstances permitted by the Family Educational Rights and Privacy Act (FERPA).
